Move progress block to very first instruction in SKILL.md
The agent was skipping the progress text because it was buried after parsing logic at line 41. Restructured so the FIRST section is "YOUR FIRST OUTPUT" with the progress block, BEFORE any tool call instructions. Added explicit "DO NOT skip this" enforcement.
